Social Work Case Manager

We are seeking an experienced social work with extensive case manager experience. This position is perfect for problem solvers and great communicators as well as task masters. If this sounds like you then read on.

Description:

In collaboration with care providers and physicians, responsible for problem solving, prioritizing and facilitating patient care objectives for the assigned area.

Requirements:

Must be L.C.S.W. eligible; M.S.W. preferred; Bachelor's Degree; BLS; one year healthcare experience; knowledgeable in state and federal regulation and crisis prevention.
