Social Worker, Adult Mental Health - London, England
Manage a diverse number of complex cases within the parameters of agreed policies and practices, including the assessment, development and review of care plans, operating as CPA keyworker for some clients, with the professional guidance and support from Senior Practitioners , in order to empower Mental Health clients by safeguarding and promoting their welfareand meeting their individual needs. Develop integrated working with the Health Service, including Acute and Community Trusts and Primary Care Groups, in addition to the development of the close working partnerships with the client, their family and carers, the Health Service, District Councils, the commercial and private sector and voluntary organisations/agencies, developing new partnerships where possible, in order to improve service delivery and produce positive outcomes for Mental Health Clients and their families. Assist the senior Practitioner and other colleagues in identifying, planning and delivering its core tasks of recruitment, assessment and training or social workers to a high professional standard. Maintain an awareness of changes in substance misuse theories, legislation, corporate and directorate policies, local and agency practicies, in order to disseminate knowledge and contribute to the delivery of a high standard service. Attend regular 'in house' core training courses and compile a portfolio of evidence supported by experiences in the field to achieve appropriate competancy levels within practice level three of the Competancy Assessment Mental Health Career Grade and study towards the Practice Teachers' Award and Approved Social Worker Award, as required by the service. Contribute to and review the development of ongoing initiatives for service development to achieve a more effective use of resources within the Mental Health Services. Ensure that information systems and client records are effectively maintained in accordance with SSD policy and joint agreement with the Health Trusts, and other levels of staff are supported, in order to provide up to date and accurate information, upon which decisions affectibg service delivery can be made. Support Approved Social Workers in carryig out their deties, to ensure that knowledge of more complex practices and procedures is gained. Requirements: • Must have a Bachelor’s Degree or Masters degree in Social Work from an accredited university. • Must have at least 1 -2 years of relevant working experience (co-op not considered) • Must be able to provide proof of certification and/or registration as a Social Worker in their province or state • Good communication skills Note: Please mention Get Social Work Jobs (GetSocialWorkJobs.com) in your cover letter when applying.
