Social Worker Needed

Provide ongoing social work counseling and case management services (including assessment, intervention, and continuity of care planning and enhance capacity) to help homeless clients cope with mental illness and daily functioning. From a managerial standpoint, responsibilities will involve program development and implementation; evaluating and clinically supervising other clinical staff; as well as collaborating with other service providers of homeless programs, the hospital, welfare agencies, homeless housing providers and area homeless shelters.

Essential Functions:

* Clinically supervising staff. Provide case management, counseling, crisis intervention, concrete assistance, assistance with process of obtaining and maintaining employment, and advocacy for homeless individuals;

* Facilitate weekly support group for homeless clients;

* Assess individual clients, using strengths-based assessment models. Facilitate client development of individual service plan;

* Assist clients during orientation and intake process;

* Provide intensive services to clients for up to six months after they enter permanant housing;

* Assist in maintenance an development of community resources to assist with multiple needs of clients;

* Complete and maintain well-organized client records;

* Assess clients applying for emergency financial assistance.
