Hospice Bereavement Coordinator

Responsibilities may include:

Providing emotional, psychosocial, and spiritual support and services to patients and loved ones before and after the death of the patient.

Working collaboratively with a team of clinicians to coordinate and provide support to meet the patient's and family's bereavement needs.

Developing a bereavement plan of care.

Maintaining detailed documentation.

Providing counseling as needed.

Organizing and scheduling bereavement groups as needed.

Developing educational materials for patients and families, staff and the community on loss, grief and coping.



Requirements for this position include:

A degree in divinity, social work, theology, thanatology or psychology.

Experience in hospice and counseling or another field dealing with bereavement.

LSW or LPC is preferred but not required.
